ABSTRACT

Covid-19 is associated with a higher risk of thrombotic complications, and prophylactic dose anticoagulants are recommended for hospitalized nonpregnant patients by international treatment guidelines. Whether the benefit and efficacy of using anticoagulants among Taiwanese patients remains uncertain, we aimed to evaluate the use of anticoagulants and to investigate the risk factors of the patients. 144 patients with confirmed SARSCoV-2 infection were admitted to a regional teaching hospital in Taipei, Taiwan between May, 2021 and July, 2021. We retrospectively collected these patients' data from electronic medical records. Descriptive statistics, independent t-test and Chi-square were applied for patients' characteristics and important factors. Medication use evaluation was performed through SOAP format for every patient by dedicated pharmacists. There were 29% patients receiving anticoagulants, 90% of the use were for Covid-19 thromboprophylaxis. Among these patients, old age, hypertension and diabetes have a high correlation with mortality and the use of anticoagulants. There was no medication related problem that remains unresolved, all patients were completely assessed. © 2022 Society of Internal Medicine of Taiwan. All rights reserved.

ABSTRACT

Since December 2019, the emergence of coronavirus disease 2019 (COVID-19) has had a major impact worldwide. This emerging virus does not only affect the respiratory system but also affects the nervous system. Cerebrovascular events have been reported as complications of COVID-19. In this review article, we summarized the epidemiology, stroke subtype, characteristics, laboratory findings, pathophysiology, risk factors and treatment of stroke in COVID-19 patients by reviewing case reports and cohort studies. On the other hand, sporadic adverse events such as systemic thromboembolism and stroke have been reported after COVID-19 vaccination. Herein, we reviewed the literature to elucidate the association between COVID-19 vaccines and stroke with the aim of assisting clinical decisions and giving people greater confidence in receiving vaccines. ABSTRACT

The modern economic era is experience-oriented, with pleasure generated through perceptual experiences to create an enjoyable process and more sales. Under the influence of the coronavirus-19 (COVID-19) pandemic, people have begun to pursue a sense of ritual and to focus on their emotion, which has enhanced the connection between brands and consumers. The new emphasis on enhancing consumer experiences illustrates how incorporation of ritual and cultural imagery have become a means through which fashion brands can distinguish themselves from global competitors. Although international fashion weeks are now being hosted through virtual catwalks, these shows lack face-to-face interactions and a sense of on-site ritual. Therefore, many have proposed that the COVID-19 era fashion industry should be redesigned to ensure shows maintain a strong sense of ritual that enables audiences to transform their perceptions through a cultivated atmosphere to experience pleasure and satisfaction. In the present study, we explored the incorporation of cultural experiences into fashion curation to identify modern design focuses for fashion curation. We also analyzed the shows of different brands participating in the fashion weeks and discussed whether incorporation of ritual in the design of the shows affected their experiential value and audience satisfaction. Our conclusions were as follows: 1. use of ritual in designing fashion shows was effective, 2. ritual in the design of fashion shows increased participants’ satisfaction with the show and its experiential value. Future studies should integrate design practice into our proposed research framework to provide a reference for fashion curation and instruction to develop curators that meet the needs of the fashion industry. © 2022, The Author(s), under exclusive license to Springer Nature Switzerland AG.

ABSTRACT

During the COVID-19 pandemic, trade measures are popular to countries to secure and strengthen the health care system's capacity. Subsidies are one of the trade measures to boost the production of medical products and build up the domestic supply chain. The article explores the legality of a subsidy responding pandemics under the framework of the Agreement on Subsidies and Countervailing Measures (hereinafter "SCM Agreement"). The analyses reveal that the uncertainty inherent to pandemic responses does not automatically exempt governments' financial contributions from the governance of the SCM Agreement. Two solutions are proposed to reserve policy space for public health subsidies. One is to reconsider the relevance of the precautionary principle in interpretation of the SCM Agreement,. another is to reflect the balancing policy in the SCM Agreement. A point central to the two solutions is harmonizing the international trade governance and global health governance in response to pandemics. Besides, the article highlights the role of regional governance in managing the emergency supply-demand imbalance caused by public health threats and exploring new subsidy rules.

ABSTRACT

Learning Objectives: 1. Provide a virtual learning experience showcasing EM for pre-clinical URM medical students with no prior EM exposure. 2. Guide students through a scholarly presentation exploring basic study design in EM specific topics. 3. Provide individualized mentorship with URM EM residents and faculty. : The EM Department at NYU Langone hosts a monthlong fully funded summer fellowship for rising second year underrepresented in medicine (URM) students from medical schools across the country. During the COVID-19 pandemic, our fellowship transitioned to remote learning to limit disease transmission. Learning objectives typically taught via in-person workshops and clinical shifts were presented in virtual presentations and interactive demonstrations. Equipment such as suture kits, splinting supplies, and wilderness medicine gear was mailed to students prior to the start date. Google classroom, Zoom, and Webex were used to facilitate the online classroom. 15 faculty and 8 residents participated through workshops, didactics, panel discussions, journal clubs, 1:1 mentoring, and Q&A sessions. Each student worked on a scholarly project throughout with their resident and faculty mentor and then presented it on the last day. The focus of the scholarly project was changed from a clinical focus to a social medicine issue in the students' local communities. We hosted 4 visiting URM students and 2 NYU students. The curriculum was rated from 0 to 10, with resident didactics (7) rated 8.17 (SD 1.91), faculty lectures (15) rated 8.05 (SD 2.20), resident simulation workshops (2) rated 8.75 (SD 1.60), and resident procedural workshops (2) rated 8.58 (SD 1.96). Every participant reported that they are more likely to pursue EM after the fellowship compared to before. This successful transition to a virtual classroom is a viable option to consider for programs seeking to continue education while reducing risk of disease transmission.

ABSTRACT

Elderly patients living in long-term care facilities have been restricted from leaving to comply with social distancing guidelines during the COVID-19 pandemic. This has led to a worsening of disorders, such as anxiety and depression. This study aims to understand the health benefits of an immersive garden experience to elderly nursing home residents with mild-to-moderate cognitive impairments. Virtual reality devices were used to provide immersive garden experiences for the residents who were unable to go outside. The heart rate and heart rate variability (HRV) data of the participants of the participants were collected using biofeedback instruments, and changes in the low frequency/high frequency (LF/HF) and the standard deviation of the NN interval (SDNN) values caused by immersive garden experiences were discussed. The results show that the immersive garden experiences were beneficial to these elderly residents. Within 6 min of completing the experiment, we found that the heart rates of participants had dropped slightly, while SDNN and HF values continued to rise. SDNN values before and after the experiment demonstrated a statistically significant improvement. Furthermore, participants expressed their satisfaction with the video intervention program. The results indicated that nursing homes can provide immersive landscape experiences to help increase HRV and SDNN of their elderly residents. This will not only help these residents recall beautiful memories of their past, but will also improve their quality of life. © 2021, International Consortium of Landscape and Ecological Engineering.
